Abstract

Introduction: Cholangiocarcinoma is a malignant carcinoma that arises from biliary epithelium. Due to its location and aggressive behaviour, management of this tumour can be difficult. The role of laboratory tests including oncomarkers is less important. Imaging methods have the main role in the diagnosis. In our opinion, cholangioscopy, when combined with intraductal sonography, offers the most precise evaluation of extent of the malignancy, infiltration depth and any growth into vascular structures.
Case report: We present the case of a 74-year-old female patient indicated for surgery based on preoperative cholangioscopy and intraductal ultrasonography; these methods were used for the diagnosis and staging of cholangiocarcinoma. Subsequently, a radical surgical procedure was performed.
Conclusion: In our opinion, cholangioscopy combined with intraductal ultrasonography is the most precise method for determining the extent of tumorous invasion of the biliary tract. We believe that it should become a standard investigation method in patients with suspected cholangiocarcinoma to determine the extent of the pathological process with highest possible accuracy and thus to assess the potential for resectability.
